Managing chronic diseases is complex for patients and healthcare providers. Patients face fragmented care plans, limited guidance, and the burden of tracking multiple health metrics, while providers struggle with time constraints and data overload, leaving gaps in care.

Patients often lack access to personalized, actionable guidance tailored to their unique health profiles. Tracking medications, diet, and activities can be cumbersome, leading to poor adherence and health outcomes. Providers face difficulties delivering continuous, individualized care efficiently, highlighting the need for innovative, patient-centered solutions.

We are developing a multimodal AI Health Advisor powered by advanced large language models (LLMs) to address these challenges. The program will analyze patient data to create tailored care plans and provide personalized diet and activity recommendations. Patients will also receive real-time, evidence-based answers to their health-related questions, improving care accessibility and engagement.

By the end of the project, we will deliver a functional prototype showcasing core features, including personalized care plans and intuitive health guidance. This tool will empower patients with holistic, actionable support while enhancing healthcare provider efficiency, bridging critical gaps in chronic disease management.
